Index bloat much? 
All of the sites I have had a look at so far (I've lost count of how many) have had problems that we would normally fix on clients' sites within the first month. Some of the site owners I've been talking to have started to implement these fixes and traffic is slooowly returning. If you are not fixing things like this, you really should be.
