Tennessee Prison Chief Faces Scrutiny Over Five Botched Executions After Christa Pike Survives Lethal Injection

Christa Pike, 50, the only woman on Tennessee's death row, remained critically ill, unconscious and on a ventilator Friday, two days after a failed execution attempt that attorneys and death penalty experts have called the worst botched execution in modern history. Pike was injected with two full doses of pentobarbital on Wednesday evening but survived, becoming the first person in the United States ever to be administered lethal drugs and live. Eyewitnesses in the death chamber reported hearing her choking and struggling an hour after the first injection, and attorneys said at least seven needles were used to gain IV access, with one bent at a 90-degree angle upon removal. Her arms were described in a court filing as swollen, burned and blistered upon her arrival at a Nashville-area hospital.
Tennessee Department of Correction Commissioner Frank Strada is now facing mounting scrutiny over his record overseeing executions across two states. Defense attorneys contend Wednesday's failed procedure was the fifth botched execution under Strada's leadership since 2022, including a May 2025 Tennessee attempt in which the procedure was called off after officials spent more than an hour unable to locate a vein in condemned prisoner Tony Carruthers, and three Arizona executions in 2022 that experts and attorneys described as prolonged and problematic. Strada served as a deputy prison director in Arizona before being appointed Tennessee's corrections commissioner. The Tennessee Department of Correction declined to answer questions about Strada's record Friday, and a spokesperson for Governor Bill Lee did not respond to inquiries.
Governor Lee suspended all executions in Tennessee through the end of the year and ordered an independent inquiry into Wednesday's events, calling the case "deeply disturbing." When pressed by reporters about Strada's history, Lee said the department of correction had "followed exactly the protocol that was outlined." Pike's legal team filed an emergency motion Friday in Davidson County Chancery Court seeking a court order requiring the state to preserve all evidence related to the execution attempt, arguing that Pike herself had warned the state and courts months earlier about her small veins, a clotting disorder and post-traumatic stress disorder that she said made a lethal injection particularly risky.
The failed execution has drawn condemnation from legal scholars, medical professionals and faith leaders, with many calling for a national moratorium on capital punishment. Bryan Stevenson, founder of the Equal Justice Initiative, said nearly a dozen botched executions in recent years should have prompted a halt to executions long before Wednesday. Sandra Babcock, a death penalty scholar at Cornell Law School who advises Pike's legal team, said the events were "unimaginably barbaric" and demanded accountability, while also calling on Lee to commute Pike's sentence to life imprisonment. Pike was sentenced to death for the 1995 murder of fellow student Colleen Slemmer and had been set to become the first woman executed in Tennessee in more than 200 years.
