That said, mistakes can be a touchy topic, so here are a few ways to minimize hurt and maximize impact:

Take ownership of the mistake, even if it’s not yours to take. This gives you a lot of power over the situation.
Assume it’s an issue with your system, not with the person. This helps you find a solution to this mistake in the future and avoid digging dungeons full of reasons.
