Update to Recommended MFA/2FA software/hardware keys

Raivo OTP was removed due to moving to a subscription model. While this isn’t an automatic “disqualification,” it was a slimy move: updating to version 1.6 silently introduced said subscription, requiring users to set up an account. Setting up an account wipes users’ existing tokens and the “restoring” tokens function is limited to subscribers.
Additionally, Raivo OTP had already been flagged on avoidthehack for some time due to its acquisition by an obscure company.
A “Preface” section was added to the post.
A new requirement was added as well: software authenticators should be able to import codes for ease of use.
